It affects the anterior wall, septum, and apex of the left ventricle. On a 12-lead ECG, this is reflected in leads V1–V4 (and sometimes V5, V6, I, aVL). 1.2 What Does “Minimal Criteria” Mean? “Minimal criteria” refers to the lowest threshold of ECG changes required to diagnose a prior (old/healed) anterior infarct. These are not acute changes (no ST elevation). Instead, they are pathologic Q waves and loss of R wave progression .
